EHR Vendors Rated on Usability Design and Testing

Only a handful of electronic health record (EHR) vendors have met best practices for user-centered design (UCD) and testing of their products under a framework created by the American Medical Association (AMA) and MedStar Health's National Center for Human Factors in Healthcare. However, the Consumer Reports–like ratings of EHRs just posted by these organizations did not compare the actual usability of those systems as experienced by the end user.

The goal of the ratings, the AMA and MedStar said in a description of the framework on the National Center for Human Factors' website, "is to draw attention to the narrow focus on only eight [usability] capabilities among the dozens required by the [Office of the National Coordinator for Health IT (ONC)] for the Meaningful Use program and to the absence of best practices in the certification process."

The partners added that ONC could use their UCD framework to improve its EHR certification program.

AMA outlined its own framework for improving the usability of EHR systems last fall. In September, MedStar researchers published a study in JAMA that found that many EHR vendors had not met ONC usability testing requirements, although their software had received certification from ONC-authorized certification bodies.

In the just-released scorecard, which rated 20 products, two Allscripts EHRs (Enterprise EHR and Sunrise Acute) and one McKesson EHR (Paragon Inpatient) received perfect scores. In the tier just below that were another McKesson EHR, Athena Clinicals, Medical Information Technology, Practice Fusion, Cerner Power Chart, Meditech Magic, and Modernizing Medicine.. Epic received only nine of a possible 15 points, and eClinicalWorks trailed the pack with a rating of five.

AMA and MedStar developed their EHR User-Centered Design Evaluation Framework "by comparing ONC's UCD certification requirements with evidence-based best practices from the human factors and usability literature," according to the website. The scorecard compares each vendor's user-centered design process, testing methodology and testing results with the evidence-based guidelines.

The framework defines user-centered design as a process that "puts the needs of the user at the forefront of the design and development, resulting in a product that is more likely to meet the needs of the user."

Low Bar of Certification

The JAMA study noted that ONC requires EHR vendors to name the UCD process they used; provide the results of usability tests, including the number, clinical background, and demographics of the participants; and make the usability reports public after the product is certified. ONC has endorsed UCD guidelines from the National Institute of Standards and Technology.

Of the 50 certified vendor reports the researchers sought, nine were not publicly available. Of the remaining vendors, 14 had not stated their UCD process, 19 used an industry standard for UCD, and six used an internally developed UCD process. There was considerable variability in the number of participants enrolled in the usability tests, and about half the vendors failed to provide the required demographic details. It was not even clear whether some of them had had any physicians test their EHRs.

"Alignment with best practices for user-centered design and testing is a starting point that regulators and industry should meet and exceed," commented Raj Ratwani, scientific director of the Human Factors Center, in a news release. "The framework we developed is the first step in bringing greater transparency to the usability processes of EHR vendors."

A 7-Point Hit List to Hiring an Exceptional Web Designer

Web design horror stories haunt every crevice of the Internet, and anyone who has ever had the itch to hire someone to build a site. Remember Wolf from Pulp Fiction? When Vincent and Jules made their mess, he was right there to clean it up, with every detail precisely orchestrated in one glorious swoop. He made the impossible seem easy.

That's how I feel sometimes. The client comes to me, and I have to Winston Wolf my way through a mess of a site, and help emotionally mend his or her shattered dreams of a perfect website. 

Asking the right questions, and having a prospective designer address your concerns up front will better arm you in making the right choice for your website direction. Here is a 7-point checklist that will guide you through your search and have you asking the right questions to find that perfect web-design partner:

1. What does his or her portfolio look like?

While having a lot of websites in a portfolio is a plus, you want to see the type of quality of your potential partner's designs. Ask to see examples of his or her work. Ask for a list of websites that are live and active so you can check out their functionality. Make sure to check on your mobile device as well.

Ensure his or her portfolio includes sites that match what you are seeking in your own site. Check out the versatility of his or her portfolio. Does he or she specialize in specific industries? Specific products or platforms?

A portfolio is important, so start there, and can typically be found on his or her website.

2. What does his or her process look like?

When you hire a web designer you want to make sure they're not going to split before the job is finished. Take some time to dig into the flow of any potential partner's process. Did he or she adequately explain how his or her design process works?

Ask for an explaination on how he or she structures projects and describe the types of management tools, workflow management and milestones used in seeing the project through to completion. A well thought-out design plan will be a confidence builder in choosing the right design team that can finish the job.

My process varies for different projects depending on the language we're going to develop and design with, if we're using an open-source content-management system, and if we're building from scratch or upgrading a current site. However, in each scenario, I have a process that I follow with tools and systems in place internally and externally for clients, and target dates for high-level milestones.

This is what you're trying to uncover when inquiring about the process. Some sort of framework they follow. This is helpful to stay on task with the target launch date, helps you follow progress and shows a level of experience that is necessary to do a successful job.

3. What software and technologies will be provided?

In my opinion, skill is subjective. You can find a very technical developer, but he or she may be lacking the skills to articulate your vision. An exceptional designer can understand the vision you have in your brain and translate it almost precisely.

Depending on the type of website you require for your business, different technology will be utilized.

Some web developers stick with a certain type of design structure or software and may not possess the type of experience with the technology you'll need. Describe the site you are looking for and ask what software he or she will use to make that happen. Discuss the options you want on the site and find out how the designer will make that happen.

Does he or she sound sure, or is he or she taking a wild stab in the dark? You may not fully comprehend all the feedback you get, but this will give you a good idea of the designer's knowledge base.

Lastly, ask if he or she has won or been nominated for any design awards. A good designer will be recognized for his or her skill by the industry. For instance, I was nominated twice by the CSS International Design Awards this year. Obviously, this isn't a deal killer, but it's nice to find a designer that has been recognized by his or her peers.

4. What is his or her vision for your project?

What is the web designer's vision for your project as compared to yours? Does he or she have a clear indication of what you want and the ability to transform that into a vision on how he or she will develop a successful website? Ask for reference sites or a mockup of his or her design plan.

Of course, you have your own vision of where you want to go with this, but the designer may have insights and perspectives that you may have overlooked. It's always good to get a good understanding of where he or she wants to take it and compare it to your own vision.

5. What does he or she do when obstacles appear?

Design obstacles can sometimes slow a project to a crawl, or even condemn it altogether. How does the designer overcome such obstacles? Have him or her demonstrate some of the sites and blogs he or she follows and explain how these resources are utilized to develop and deliver a final product.

Run a search on various platform sites where he or she provides expertise, such as Stack Overflow, Quora, Github and Doctype, which provide programmers and designers a forum from which to collaborate on design concepts and solutions. If your potential designer is a common users of these sites, there will be links to his or her contributions.

I'm involved in multiple Facebook and Slack groups for web designers. If I run into issues, this is typically where I turn for advice. It's very helpful to have a large pool of resources and peers I can bounce ideas off.

6. Is he or she up to date with the current trends and designs?

Web design is constantly changing. Less than six months ago, flat design was in and parallax was fading out. Recently, I've seen a surge of parallax design and flat design fading out. Mind you, this is my perspective. Others may not see trends this way. This is a perfect example of perspectives and how the design process is subjective.

Ask about current design trends and review the designer's portfolio to make sure he or she understands what's current. Most of the time, a good web designer will help you flush out your vision of the site and provide insight to what's current.

7. Should you give direction or not be picky?

Everyone who wants a new website usually has a unique vision or direction for its design and functionality. You can choose to drive the project or you can allow the designer to use his or her best judgment. Web design is subjective and many times the design you envision in your head isn't what the designer will have. Does he or she listen to what you want, or talking over you to "tell" you what you want?

Provide reference sites for any potential designer and guidance in your expectations. You don't want someone to just tell you what you want to hear. For an undertaking such as this, you want a true partner that will work with you every step of the way and ensure your demands and expectations can be reasonably met or exceeded. You can make the choice to provide guidance for the entire project, or simply provide input for parts of it and let his or her expertise guide the rest.

There's always a margin of error in web design, no matter who is designing and how skilled that person is. Getting answers to all of your questions and concerns on this checklist is an important first step in finding the right design partner.

Nearly 10 percent of online donations are lost due to bad nonprofit websites

According to a study from Nielsen, the online donation process is less user-friendly than typical e-commerce transactions. What Legos Can Teach Us About Software Design

I loved playing with Lego as a child. Once I would build spacecraft, vehicles or castles according to the directions, then I'd repurpose the Lego pieces to create new models unimagined by the designers.

Lego attracted generations of customers primarily because they do not force us to adhere to a strict system of what you can build and what you can't. However, this is not the norm for many of our other human tendencies. We seem to have an instinctive need to organize the world into strict hierarchies, like the Dewey Decimal System, the taxonomy of species and the pages of Wikipedia. The problem with strict hierarchies is they work in their existing environment for predetermined purposes. But they fail when new, disruptive creations come along that don't fit that categorization.

We see this in software design. Decades after playing with Lego as a small boy, I realized that the same open, flexible design pattern also makes for great software platforms. Because the most powerful software must adapt to changing environments and needs in markets that didn't exist when the software was designed.

Here is the basic approach to open design in software: First, develop an open, flexible framework that can take forms or perform functions defined by its users (not the software vendor). Then ship it with some built-in "happy path" templates or usage scenarios that allow customers to be productive and solve problems immediately. But don't hard-wire these templates and usage scenarios. When customers come across new and novel problems to solve — ones that were never even envisioned by the original developers — provide extensive training and support to extend the behavior of that framework.

The largest Web companies in the world follow this open design pattern. It's also why they invest in open source software. Not because it's free of costs, but because open source software can be redesigned by companies to fit their own special needs, and needs unimaginable today. Compare and contrast this to the software frameworks of the 1980s and 1990s, which looked a lot like the Dewey Decimal System. Produced by closed-source vendors, they forced you first to absorb their worldview and learn proprietary tools and languages to develop models within rigid hierarchies.

The Lego-like software design pattern — flexible, pragmatic frameworks that allow you to easily achieve a goal but also can be extended in innumerable ways — could not have happened without the rise of the open source movement. Without open source, the breadth of the happy path use cases would be limited. Developers would invariably have tended toward more prescriptive, inflexible patterns that fit whole industries at best, but certainly not the emerging needs of specific companies.. Nurturing a broad open source community with diverse viewpoints is key to developing software that works easily for most people, but is also suitable and usable by experts.

The company where I work, Chef, built an automation framework by the same name. Chef, along with other companies in its sector, including Docker and Puppet, are practical examples of how this design pattern has proven its worth in the marketplace. When we first started Chef in 2008, cloud infrastructures had barely gotten started, containers didn't exist and continuous delivery was used by only a handful of practitioners (the book wasn't written until two years later). But the Chef automation framework was built to be extensible and adaptable, and over time, our customers have been able to solve problems that were unimaginable in 2008, an outcome that would have been impractical with a rigid, proprietary toolchain.

We can expect to see more of the same in the coming decades. As every industry competes online, enterprises of all shapes and sizes are facing the same challenges that large Web companies first faced. Traditional enterprises are finally becoming mature participants in the open source ecosystem, not only permitting employees to use open source software in their day-to-day development work, but contributing tools and libraries back to the community. The next stage of evolution is for those same enterprises to allow developers to spend time curating open source projects, and also developing industry-specific communities of practice that can band together to create open software platforms for those sectors. No longer should common components and libraries be seen as a competitive advantage, but as building blocks — Legos, even — for companies to create awesome user experiences mediated through software for their customers.

Just as nobody looks at a grand Lego creation like a 20-foot Tyrannosaurus rex and sees the individual blocks, these software frameworks too will become part of the undifferentiated heavy lifting necessary to get a product to market. The differentiating factor between applications will be only in consumer-facing features, and the delight a customer feels using them.

Mechanical Engineers Release Web-Based 3D Tool for Part Design Optimization

Thanks to a team of mechanical engineers at the University of Wisconsin-Madison, design engineers now have Web access to 3D design-optimization software that will help them more quickly create parts that are cheaper to produce and weigh less.

The engineers—led by UW-Madison mechanical engineering Professor Krishnan Suresh—developed the software program called ParetoWorks that's been available as a free plug-in for SolidWorks since 2013. But now they've released a Web-based version, making the software—which researchers said some users have called a "gift from heaven"—more widely available.

"We've been getting a lot of positive feedback," Suresh said of the software, which allows designers to optimize part design for myriad products, vehicles, and devices—from bicycles and airplanes to bridges and furniture. It does this by helping them quickly identify how component shapes, or topologies, maintain their structural integrity while using the least amount of material possible.

This design optimization "lies at the heart of modern engineering," Suresh said, because by using fewer materials, parts are both less expensive to produce and weigh less while increasing quality. These are the key goals of design engineering and critical to innovation today, he said.

While the design optimization ability of the tool is not new, the speed with which it can identify an optimized shape for an object—in mere seconds—is much faster than other available software and beyond what most humans can do, he said.

"To the best of our knowledge, this is the first implementation of a full-fledged 3D cloud-based design optimization," Suresh said. "It's an accomplishment that even large corporations are struggling to match."

The National Science Foundation, Sandia National Laboratories and Autodesk include those who have funded the work of Suresh and his team. The engineers also have been working with local design companies such as Madison-based Design Concepts to continuously improve the product.

The next goal for the software is commercialization, Suresh said. For that the team has partnered with Jon Eckhardt, a professor at the Wisconsin School of Business and the executive director of the Weinert Center for Entrepreneurship at UW-Madison, to explore creating a spinoff company.

How scary is a blank sheet of paper when it's the only thing standing between you and that ideal gig? You are putting too much pressure on yourself, especially if you're not accustomed to the business side of being a designer.

    To get started, just create a colour palette in either Photoshop or Illustrator (or any other design software you use) and save it under the specific brand's name. Next time you create a brand new document, these palettes will be accessible without having to open any other file. You can also get a headstart with some pre-made swatches.

    These swatches, for example, are colours frequently used in flat design. Adding the swatch file to your Photoshop document makes it much easier to use them anytime you need to.

    Once you've created a style guide, you can easily share that with the rest of your team so that everyone can stay on brand. You can also create a coded styleguide that's a living web document. Or you can even create a custom library of elements, like the ones found in UXPin, that can be shared amongst the team.

    05. Actions

    Let's say you are working on an ongoing project where there's a defined visual feel. Maybe that "feel" is a specific texture, colour tone or brightness level. Regardless, record a set of Photoshop actions that you find yourself constantly redoing.

    Actions are sequences that become repetitive in your work, and you'll basically store them to save a few minutes every time they become necessary.

    06. Keyboard shortcuts

    Finding that you're using "Paste → Special" too often? Is there a certain phrase that you keep repeating to open or close emails? Are you a Pen Tool person?

    Automate your keystrokes to reflect your most commonly used functions. There are many ways to go about this, but I'll describe two.

    You can use your design software's preset keyboard shortcuts to accomplish certain things faster. Can't remember what those are? Here's a handy cheat sheet for Photoshop CC:

    Alternatively, if your thing is text snippets that repeat, consider a tool like TextExpander.

    You can set up custom chunks of text that show up every time you type a certain abbreviation.

    For example, if I wanted to avoid typing "Best Regards, Laura" every single time, I'd create an abbreviation like ";br" and the entire thing would be pasted on my text editor (word processor, email, etc).

    07. File management

    OK. The file chaos has to stop. Get your stuff together and take some time to organize your design assets and project files in folders that make sense. This is a complete time-saver in the short and long run.

    Make full use of Dropbox to help organise your files

    To save even more time, download an app like Dropbox for desktop and work directly on each file. Your work will automatically synchronize via cloud storage and it will be immediately available to share with others.

    On top of that, if you're working on a shared folder, where someone else already has access, Dropbox notifies that other member that something is being updated so you don't have to.

    08. Prototyping tools

    How many times have you jumped into a full-fledged design project without questioning the concept for a minute?

    How many times have those decisions resulted in expensive, and quite tedious, changes later on? Prototyping helps you build an early version of your design solution so that you can gather feedback and decide what the right direction is.

    Tools like UXPin make an interactive prototyping a website a matter of a few clicks. Wireframing becomes simple when you only have to drag and drop pre-designed components into a simple grid.

    Once your prototype is finished, the tool allows you to share what you've built with colleagues or clients who can comment anywhere on the page.

    The bottom Line

    There is always a better way to do things. Find it.

    The Essential Website Design Checklist for 2015 and Beyond

    I recently had someone ask me for a definitive checklist for designing a website. We had already published an On-Page SEO Checklist, which is helping hundreds of readers every month who come looking for a definitive checklist that will help them perfectly optimize their posts and pages.

    I recently had a frustrating experience searching for a web design checklist, which I couldn't find, so I took to the Internet to track it down. There were a number of results, but understand my frustration when I couldn't turn up a single site offering me a complete checklist.

    Considering this, I decided it was about time to make a website design checklist that will be the essential guide for web designers, freelancers and agencies starting web design projects. This article will provide a checklist that will help you in your next web design project.

    1. Project Goal

    Before you write even a single line of code, identify and establish your goals for the project. Defining a purpose will put things into perspective and will speed things up.

    Ask yourself the purpose of your website–your website needs to center around specific business goals. Do you want to engage with your audience through your platform or you are just building it to showcase your portfolio? Do you want to generate leads or is your goal to sell products?

    Write down your goals and build everything with these key objectives in mind if you are managing the project yourself. If you are working with a client, make sure to send them a questionnaire that will help you understand their goals and will help streamline the project.

    2. Key Performance Indicators

    Gary Vaynerchuk recently shared the following that resonates with me on one of his Facebook posts, and I wanted to share it here so you know why it is important to set the right expectations for your personal or clients' projects: One of the hardest parts of working in client services is getting the client to actually tell you what they really want.

    This is extremely important because you don't want your designs to keep coming back for more edits and revisions. I understand that some clients are just difficult to work with, but some just need to be asked the right questions, because without the right questions, you won't get the right answers that will set the wrong expectations for the project.

    Set the KPIs of the project; these include delivery time to expected results, such as direct sales or leads. They can even be generating press if you are designing a one of a kind website. Identifying your KPIs will help you measure the ROI at any time, but more on that later.

    3. Framework

    Identifying your website's framework is the most important decision you'll have to make before kicking things off. By default, a lot of platforms have limitations or they are just too complex (e.g., WordPress or Magento). Pick the framework you'd like to use for your website.

    As we are on the topic of setting up the right foundation, I want to touch on the topic of picking the right web hosting for your business. I've had my fair share of trouble with various hosting companies and recommend the following web hosting companies in order:

  • SiteGround or WP Engine
  • Bluehost
  • InMotion
  • There are expensive options available too, but if you are just starting out, both SiteGround and WP Engine offer a feature called "Staging" which lets you create a replica of your live site, make changes, and with a click of a button copy your staged site on the live site.

    Having said that, you can always go for more expensive options, but the ones I listed are the one I use and recommend.

    4. Development

    What does your buyer journey look like? From start to finish, visualize how and where customers will come from and the tasks they will want to perform on your site.

    Do the visitors want your contact information, your physical address, a description of your products or services, reviews from other customers, or the ability to purchase your products online? Profile your website visitors and their needs, and base the structure of your website around that.

    Also ensure your website will be responsive or adaptive. Validate HTML, CSS, and other scripts and ensure they are properly optimized so that the website performs smoothly right off the bat.

    5. Launch

    Before pushing the "Go Live" button, test the site using real devices or use services that help you check your website in various environments.

    6. Track and Measure

    Now that your website is live, use the testing services, software, and scripts mentioned above to track and measure the behavior and performance of your website. The KPIs and objectives that were established earlier will help you see what is going on with the website and if there is room for improvement.

    Now that your website is ready, get ready to market it.

    4 design rules for branded digital experiences

    The overall quality, accessibility, and dynamism of a company's digital experiences can make or break its brand. Luxury brands, in particular, know this reality all too well.

    Wolf-Gordon, for example, is a design-focused B2B wallcovering company that came to the realization that not only was its website not providing a digital experience that users found worthy of engaging with, but it was also not branded well. Rather than conveying the company's focus of "designing as the driver", its website content was utilitarian and rote.

    When Wolf-Gordon, a client of ours, and karlssonwilker, its design firm, analyzed the competitive landscape, they noticed that there was little that differentiated the company from its competition online.

    If the brand didn't look interesting or relevant online, how well could its products lead the market?
